Assumption of Risk Form You will be required to sign an Assumption of Risk form which states the following: I agree that I will not take any legal or illegal substance before or during the program that may affect my judgment or physical responses during the program. I am aware that during my participation in any Urban Descent activity, certain risks or dangers may occur which may include, amongst others: • Physical exertion to which I may not be accustomed • Extremes of weather and temperature including sudden and unexpected change • The hazards of participating in activities at height • The possibility of accident or illness • Difficulties of evacuation if I am injured or disabled I acknowledge that the enjoyment and excitement of Urban Decent activities are derived in part from risks incurred by participation in those activities, and that these risks may exceed those commonly accepted at work or at home. I accept all the abovementioned risks of my program, and the possibility of personal injury, loss or property damage resulting from, and will hold Urban Descent, its employees, contractors and agents free fromm any and all liability, actions, debts, claims and demands of any nature whatsoever, subject to the rights and privileges which apply under the Fair Trading Act (1984), the Trade Practices Act (1974), & the Civil Liability Amendment Act 2003. I grant Urban Descent, its employees, contractors and agents, authority to use photographs taken during this program to be used in promotional and advertising material for Urban Descent’s and or the represented charity’s purposes. These photographs will not be on sold to any third party and will only be used in the promotion and advertising of like events. In entering this agreement I do so only of my own free will.

Sir David Martin Foundation was set up in 1990 by the much loved Governor of New South Wales, Sir David Martin, four days prior to his death. The Foundation aims to realise the dream of Sir David Martin of an Australian community “where children can grow up in safety with hope and opportunity”. The Foundation raises funds for innovative and effective Mission Australia run youth crisis programs. We provide care, counselling, training and support for young people in crisis and foster community understanding of their needs and potential. With the goal to get more young people off the streets, away from unhealthy situations and into care and rehabilitation. We aim to encourage them in a new sense of self belief so that they can face the community at large without the need for drugs, crime and homelessness.
